My Bone Scan Says Osteopenia - Should I Start Medication Now or Can I Wait?
I still remember the way the technician said "all done" and how quickly the room emptied, leaving me alone with the hum of the machine. Two days later, my family doctor called. The pathophysiology is multifactorial: estrogen decline, calcium and vitamin D insufficiency, secondary hyperparathyroidism, chronic inflammation, malabsorption, and mechanical unloading all play roles (Compston et al., 2019). A DEXA scan captures the endpoint of those processes, not the processes themselves. When you are seen only through that endpoint, the only options appear to be "wait" or "medicate."

Modern medicine
The person from modern medicine looking at you is looking at your bone mineral density, your fracture risk, your hormone status, and your secondary causes of bone loss—and they would pursue: a DEXA scan of the hip and spine, a FRAX score to estimate your ten-year probability of a major osteoporotic fracture, blood tests for calcium, phosphate, alkaline phosphatase, vitamin D, parathyroid hormone, thyroid function, and sometimes a bone turnover marker. They would ask about your history of fractures, your family history, your smoking and alcohol use, your medications (especially glucocorticoids and proton pump inhibitors), and your menopausal status. The direction of adjustment is to reduce your fracture risk by correcting reversible causes and, in selected cases, starting an antiresorptive or anabolic medication.
The evidence for this approach is strong in osteoporosis but more nuanced in osteopenia. Bisphosphonates reduce vertebral and non-vertebral fractures in postmenopausal women with osteoporosis (Black et al., 1996), and guidelines recommend treating osteopenia only when the FRAX score crosses a certain threshold or when other risk factors are present (Kanis et al., 2019). It should be noted that the evidence for treating osteopenia pharmacologically in people without a prior fracture is much weaker, and the absolute risk reduction is often small. Many people with osteopenia are advised to wait and monitor, which is a legitimate strategy but one that leaves the underlying drivers unaddressed.
Traditional Chinese Medicine
The person from Traditional Chinese Medicine looking at you is looking at the quality of your Kidney Qi and your Kidney Essence, the strength of your Spleen and Stomach, and the flow of Qi and Blood through your channels—and they would pursue: questions about your lower back, knees, teeth, hair, hearing, and urination, because in TCM these are all governed by the Kidney system. They would ask about your digestion, your appetite, your stool, and your energy after meals, because the Spleen is responsible for transforming food into Qi and Blood, which then nourish the bones. They would look at your tongue and pulse, and they would ask about your sleep, your stress, and your menstrual history. The direction of adjustment is to tonify the Kidney, strengthen the Spleen, and move Qi and Blood so that the bones receive nourishment.
The evidence for TCM in osteoporosis is promising but preliminary. A systematic review of Chinese herbal medicines for postmenopausal osteoporosis found that some formulas, such as those containing Epimedium (Yin Yang Huo), were associated with improvements in bone mineral density compared with calcium alone, but the trials were small and often of low methodological quality (Wang et al., 2013). It should be noted that most of these studies were conducted in China, used different formulations, and did not report fracture outcomes. TCM is best understood as a supportive approach that may help some people, not as a proven substitute for fracture-reducing medication.
Ayurveda
The person from Ayurveda looking at you is looking at your dosha balance, your agni (digestive fire), your dhatu (tissue) quality, and the accumulation of vata in your bones and joints—and they would pursue: questions about your digestion, your elimination, your sleep, your anxiety, your joint cracking, your dry skin, and your tolerance for cold and wind. They would assess whether your vata is aggravated, which in Ayurveda is associated with dryness, lightness, and degeneration of the asthi dhatu (bone tissue). They would also look at your ojas (vitality) and your history of overwork, travel, and irregular routine. The direction of adjustment is to pacify vata, strengthen agni, and nourish the bone tissue through diet, herbal formulations, oil therapies, and daily routine.
The evidence for Ayurvedic approaches to osteoporosis is limited. A small randomized controlled trial of an Ayurvedic formulation containing Withania somnifera, Terminalia arjuna, and other herbs showed some improvement in bone mineral density in postmenopausal women, but the sample size was small and the follow-up was short (Sharma et al., 2010). It should be noted that Ayurvedic herbal medicines can interact with conventional drugs and should be used under supervision. The tradition offers a rich framework for understanding constitutional patterns, but it does not yet have the large fracture-outcome trials that modern medicine relies on.
Mind-body / Stress physiology
The person from mind-body and stress physiology looking at you is looking at your cortisol rhythm, your sleep architecture, your autonomic nervous system balance, and your history of chronic stress—and they would pursue: questions about your sleep quality, your morning energy, your heart rate variability, your history of trauma or prolonged caregiving, your caffeine and alcohol intake, and your exercise patterns. They would also ask about your relationship to your body and your beliefs about aging. The direction of adjustment is to restore a healthier stress response, improve sleep, and reduce the chronic sympathetic drive that can impair bone formation.
The evidence here is growing. Chronic stress and elevated cortisol are associated with lower bone mineral density and increased fracture risk (Wippert et al., 2017). Mind-body practices such as tai chi and yoga have been shown to improve balance, reduce falls, and in some studies modestly improve bone density in postmenopausal women (Wayne et al., 2012). It should be noted that these studies are often small, and the effects on fracture risk are not yet established. Mind-body approaches are best used as part of a broader plan, not as a standalone cure.

What is osteopenia, and is it the same as osteoporosis?

Osteopenia is not a disease; it is a risk category. Many people with osteopenia never develop osteoporosis, and many never fracture. Should I start medication now if my T-score is -1.8?
That depends on your overall fracture risk, not just your T-score. Tools like FRAX estimate your ten-year probability of a major osteoporotic fracture using your age, sex, weight, height, previous fracture, parental hip fracture, smoking, glucocorticoid use, rheumatoid arthritis, secondary osteoporosis, and alcohol intake. Can I wait and see if my bones improve on their own?

Waiting is not the same as doing nothing: it should include regular follow-up, repeat DEXA scans every two to three years, and a plan to reassess if your T-score drops or if you have a fracture. Do I need calcium and vitamin D supplements?
Calcium and vitamin D are essential for bone health, but more is not better. Most adults need 1,000 to 1,200 mg of calcium per day from food and supplements combined, and 600 to 800 IU of vitamin D, though some people need more to reach a blood level of 30 ng/mL. High-dose calcium supplements have been associated with kidney stones and possibly cardiovascular risk, so food sources are preferred. Vitamin D deficiency should be corrected, but mega-doses are not helpful and may be harmful.
Can Traditional Chinese Medicine or Ayurveda reverse osteopenia?
There is no reliable evidence that any traditional system can reverse osteopenia on its own. Some small trials suggest that certain herbal formulas may be associated with modest improvements in bone density, but these studies are preliminary and do not show fracture reduction. TCM and Ayurveda are best used as supportive approaches alongside conventional care, not as replacements. Is exercise safe if I have osteopenia?
Yes, exercise is generally safe and beneficial, but the type matters.
