I Was Told Lupus Has No Cure — Does That Mean Nothing Can Help Me?

The rash across my cheeks came first, then the joint pain that made me feel eighty at twenty-five. For decades after hydroxychloroquine became standard of care, the treatment options barely changed: corticosteroids and broad-spectrum immunosuppressants — cyclophosphamide, mycophenolate, azathioprine. These drugs save lives and organs, but at a steep cost in side effects. Between 2011 and 2021, three genuinely new disease-modifying therapies received approval: belimumab, the first biologic ever approved for SLE, which targets B-lymphocyte stimulator and reduces disease activity and flare risk (Furie et al., 2011); anifrolumab, a type I interferon receptor antagonist that showed significant reduction in disease activity in the TULIP-2 trial (Morand et al., 2020); and voclosporin, a calcineurin inhibitor that, when added to standard therapy, nearly doubled renal response rates in lupus nephritis compared to placebo (Rovin et al., 2021).

This is not a miracle. These drugs do not cure lupus. They do not work for everyone. They come with their own risks and costs. But together they represent the first wave of targeted therapies in SLE history — drugs designed around the actual immunological pathways that drive the disease, not borrowed from transplant medicine. "Nothing we can do" can be crossed off the list.

1. Can lupus actually get better, or is this permanent?

But here's what we can tell you: the treatment landscape for lupus has changed more in the last fifteen years than in the fifty before them. People who were on high-dose steroids for years — with all the weight gain, bone loss, and infection risk that implies — have been able to taper down significantly after adding targeted therapies that didn't exist a decade ago. Long remission — defined as no clinical activity and minimal or no steroid use — is achievable for a meaningful proportion of people, though it is not yet achievable for everyone. 2. Is lupus fatal?
