Why Is My Postherpetic Neuralgia Worse at Night — and What Can I Do About It?

At night, several things converge: descending inhibitory pathways from the brainstem are less active during sleep transitions, sympathetic tone shifts, and the absence of daytime sensory input means the brain has less "competition" against the pain signal. This is why the same nerve that was tolerable at noon can feel unbearable at midnight (Baron et al., 2010; Johnson & Rice, 2014). The standard medications target the nerve and the spinal cord. They do not target the clock, the bedroom, the stress response, or the meaning of the dark. That is the gap.

Evidence: A 2019 systematic review and meta-analysis of acupuncture for postherpetic neuralgia found that acupuncture was associated with greater pain reduction than conventional medication alone in several small trials, but the authors noted high heterogeneity and risk of bias (Wang et al., 2019). It should be noted that most TCM trials for PHN are small, unblinded, and conducted in a single country, so the evidence is promising but not definitive; acupuncture is generally safe when performed by a licensed practitioner.
Ayurveda

Why does postherpetic neuralgia get worse at night?
At night, several factors converge. Descending pain-inhibitory pathways from the brainstem are less active during drowsiness and sleep transitions. Daytime sensory input — noise, movement, conversation — competes with the pain signal and partially masks it. When the environment goes quiet, the brain has less to attend to, so the pain occupies more of your awareness. Sympathetic nervous system activity also shifts, and for some people, the warmth of bedding or the pressure of sheets on allodynic skin adds a direct trigger (Baron et al., 2010).
Is nighttime PHN pain a sign that the shingles is coming back?
